It's easy... just print the picture onto iron-on paper (make sure it's for dark shirts if you dont' do a white shirt), then iron on! All done! :) I buy the really cheap shirts from Walmart and the paper... costs like $5 to make a tee.
I've done a Muppets record album cover on a red tee, a bumblebee on a teal tee, the Seasame Street sign with the words in Russian on a spring green tee with a pic of all the international characters on the back... I did all of our Halloween tee's for work. We were super heroes and evil villans and I made characters for everyone. :) It was awesome. I did Mark Henry's first book cover on the back of a black tee with a name tag on the fron that said 'My Name Was Plaguecarrier #1' and made one for Mark too, though I don't remember his name... but we liked the 'was' since it's a zombie book. Um... yeah, those are just the ones I wear all the time.
